What is needed for direct billing
INSURANCE CARD
An insurance card shows the insurance company, name of the insured member, policy number, member ID and if the cardholder is the insured member or a dependent. All of this information is needed to submit a claim.
DOCTOR’S NOTE
Only if a doctor’s note is needed to validate the massage claim.
eCLAIMS CONSENT FORM
Depending on the insurer, a 3rd party billing form may need to be signed to allow direct billing. Sun Life and insurance companies under Telus Health need a consent form signed.

Insurance companies do not share information regarding your policy to anyone.
Due to patient confidentiality, clinics and health care providers are unable to check balances, statements, limits and the policies of your benefits plans. You are the only one who can get that information.
When you have more than one benefit plan.
Only your primary plan can be directly billed. A receipt will be emailed with the amount covered by your insurer and the remaining balance that was paid out of pocket. This receipt can then be submitted to your secondary plan.
