Booking and Payment

Book an appointment by email (eleanor@elevatingtouch.com) or by phone (437-436-0699). Texting is preferred as I am unable to pick up calls while working. A courtesy reminder via text and email will be given a day before your appointment.

Payments are due at time of service either before or after your treatment. Payment options include using your benefits (direct billing is available), cash, or e-transfer.

If you need to reschedule or cancel your appointment please give at least 24-HOUR NOTICE. Anything less than that will be inadequate time to offer your appointment to others.

Please call to reschedule if you’re sick because massage may worsen your condition and you may put others at risk.

Late Attendance Policy

Everything will be prepared and set up so that your session starts on time. It is your responsibility to arrive on time. If you arrive late, your session will still end at the designated time.

Rescheduling Policy

If you need to reschedule please do so as soon as possible. A 24-hour notice is needed to see if another client can be accommodated during your time. If less than 24 hours is given a $10 fee will be added to your next appointment.

Cancellation Policy

If you need to cancel please do so as soon as possible. 24-hour notice is needed to see if another client can be accommodated during your time. If less than 24 hours is given a $20 fee will be added on to your next appointment.

No Show/Missed Appointment Policy

If you have not arrived by the halfway point of your session it will be considered a missed appointment and the full fee for your session will be added to your next appointment.

Consent Policy

Your consent is needed to collect your personal information (including your health history). Your information is needed to ensure that it is safe to provide massage therapy and to be able to contact you regarding your appointments. Your consent is also needed to continue with assessments and treatments. Indications for assessments, treatments, and the use of certain techniques and modalities will be explained during the session. The potential risks, benefits and draping used will also be discussed to allow you to make an informed consent. Special consent is needed whenever there is a clinical reason to assess and/or treat specific sensitive areas (anterior chest wall, breasts, upper inner thighs, gluteal regions). A consent form will need your initials and signature dated every time special consent is needed.

Your consent to collect personal information, for assessment, treatment, and the use of certain techniques and modalities can be withdrawn at any time during your session. However, if you decline the consent to collecting personal information you cannot be given treatment.

Privacy Policy

In accordance with provincial privacy legislation your informed consent regarding the collection, use and disclosure of personal information needs to be obtained. The collection, use and disclosure of personal information is only to the extent necessary for the service (massage therapy) provided. Your information needs to be updated annually or redone if it has been more than a year since your last visit to ensure that your file is up-to-date. Any information related to you will not be disclosed without your written authorization.

Efforts are made to ensure that your personal information is secured. Information recorded on paper is kept in locked and secured files. Information collected electronically is secured in applications and devices that are password protected. In the case of discarding information, paper records will be shredded and electronic records wiped out and the hardware physically destroyed.
